    I get the feeling that what happened might have been that in one month OH got paid from both companies at the same time (maybe changed jobs half way through the month or was waiting for holiday pay) and that is why BR was applied...
    But then never changed with the arrival of the P45.

    It would make sense.

    Did your OH receive this?

    He never received the P11d form from this employer (which he has now left). This was the first job in which he had received benefits, and at the time he didn't realise they were supposed to supply this form to him.

    I am assuming that the company supplied this form retrospectively to HMRC (but still not my OH) which is why this came to light at this late stage?

    BAA1: the P45 shows his tax code at the previous job was 503L.
