Default Removal - Template Letter

Hiya 
I have a default for £60 on my Equifax file from Virgin Media from Jan 2009.
I had no idea that this was on my credit file until I checked it the other month.
I had been a customer with Virgin media on a Direct Debit and then attempted (and apparently failed!) to cancel it when I moved.
I've never received a default notice from them and it's such a small amount as well.
I've paid the amount and been given a reference number and when I spoke to someone there they said I had to contact Equifax myself to remove it. I've called back and been given an address to write to to try and get it removed.
I was just wondering if anybody had a template letter or something that I can send off?
The strange thing is it's only on one of my credit reports and not on the other one!
Any help would be greatly appreciated as I'm in the process of trying to get a mortgage and this has put a bit of a spanner in the works.

Hi there,
Unfortunately defaults, if correct, cannot be removed. They can only be marked as 'settled' once they have been paid.
Therefore I suspect if you write to Equifax asking them to remove it they will refuse as they have a duty to provide an accurate picture of your credit.
Does it say on your credit file that the default has been settled? If not write to Equifax with proof that you have paid and they will get it corrected for you.
