How to fill in a N349 (Third party debt order)

Hi all, I am in the process of getting my money back from a cowboy plumber..I have a judgement in my favour and his bank details so I want to fill and file in a N349..
I have printed in the form and have been looking around for a guidance sheet on how I should fill in the form...
Would any one know where I can find one or point me to a 'sample' so I can see how to fill it in..
Most of it I do get but there are bits at the top right of form which look 'official' and I am not sure I am suppose to be filling them in or if it is the court that will do it
Also where do I post the thing? There is no address on the form itself :-s

Claim no: (is the ref no you should already have.)
Leave 'Fee account' and 'Appn. no.' blank
