A few questions about survey sites & tax

Hi i signed up to a few survey sites a couple of months ago and i'm just filling out the tax form. I was going to phone them but i cant do that now and i'm getting a bit confused figuring out all the information, i know i'm over complicating things for myself!
I just wondered, does the money that i've earned on the survey sites count if i havent withdrawn it yet or if i havent reached the payout threshold yet?
Also, I know the income i get from the surveys will be way, way under £5075 but i still have to fill in a Class 2 national insurance contributions (NICs) form anyway? I don't really understand what it is.. :doh:
