Phoenix Recoveries - help please

Hi I am new to this so please bare with me if I have posted in the wrong place.
I recently obtained a copy of my credit file from Equifax to find that the above company have registered a default against me for £105. It says that the start date was 08/01/2003 and date updated was 25/01/2009.
I placed a dispute with Equifax as I had no idea why a company I had never heard of had registered a default against me. I received a response stating that the company had investigated it and that the information should remain unchanged. Phoenix recoveries said it was from a Choice for You (mail order account) and that a default notice would have been sent to me.
I have moved house 3 times in the past 4 years and have therefore never recived any such notification. I remember having a catalogue account with Choice for You but when I called Choice (company has since been renamed) they had no records of me or my old account.
How can I go about getting this removed from my credit file? It is so frustrating because over the past 6 years I have paid off my debts and everything else is showing up as green. I would like to apply for a mortgage this year and am fearful my application will be rejected.
Is there some sort of letter template for this kind of thing?

What was the "default date" listed on the report?

It say's date last delinquent 11/2006 but no specific default date.
I have worked incredibly hard to clear a massive bank loan, credit cards and catalogues but my credit rating is still low - ish (about 350). I want to get rid of this off my credit file and move on.
Is there anything I can do? When will it 'disappear' from my file? Are my partner and I likely to get accepted for a mortgage?
Has anyone got any suggestions for what I could include in a letter to Phoenix (who apparently are notoriously difficult to deal with)?
I would be grateful for any suggestions. Thanks.0
