you can self certify your illness for up to 7 days. Most employers are happy with you just telling them but if they insist on a note, you can ask your recptionist at your doctors surgery and they will have a form you can fill in youself.0

Some employers require a med cert. In this case it will normally be a private note, and chargeable.
OP, if you do need a med cert you will need to ask the doctor and see what they say. They may need to do further tests, but that would depend on your condition.Gone ... or have I?0 -
I am supposed to provide a doctors note after 4 days, not 7. So now i have taken 4 days off so far because i think i have some sort of stomach bug and fever and have made a doctors appointment.
Also i am on a future jobs fund contract and have 3 weeks left.
Does anyone know if they can end my contract early because i am sick for a long period of time? and would i avoid a sanction?0 -
Btw does anyone know how much a sick note would cost me?0

Private sick notes are £15 at our surgery,
It would appear that they are now called Fit Notes!
Charges for fit notes
Some employers may request a fit note, for example, from employees who repeatedly take time off sick, even if, each time they’re off work, it’s for seven days or less.
For sickness of seven days or less, your GP practice may charge you to provide a fit note. Your GP practice can tell you what their charges are.
