No application form but there is a paragraph next to the token (on page 50):
To claim your William & Kate plate, send your 50 tokens and a cheque for £2.75 to cover postage and made payable to 'Daily Mail' to: PO Box 5002, WKO, Alton, Hants., GU34 9BZ.
