Dental Hygienist Treatment NHS?

If nhs band 1 treatment is not useful then nhs band 2 treatment for advanced gum problems will be. If the practice is offering nhs and private treatment they must issue a fp17dc form with details of costs.0

Here's an update - saw the hygienist for treatment who suggested I visit 2 or 3 more times as there was "quite a lot" to do and mentioned 2 to 3 mm to remove (is that serious or not ?). The hygienist mentioned one of my teeth probably needed a filling (no mention of tooth removal). At the moment I have no symptoms and the situation didn't sound as severe as the first dental diagnosis. Wasn't provided a FP17DC form.
Any thoughts ?0
