Once you've registered on the contact form, click on the OP's orginal link and re-enter your details and you should get a "Thank You" page, which confirms that you will get one. I don't think that just completing the contact page will work.

I also had to submit the contact form to be able to request the notebook - was a bit unsure about what to write in the message field though 'only registering to get free stuff' :rotfl:
