Bankruptcy forms: How much do I owe?

Hi,
Please can someone help me...
I have had my head in the sand for a long time but am now (finally) about to file for bankruptcy, but I don't know how much I owe and to which companies (have been ignoring phonecalls and letters for about 3yrs).
Please don't have a go at me I know this was REALLY stupid, and have been/am Very stressed out about it all.
I thought I could get my credit reports, but it sounds like these aren't always correct.
I have been sent loads of letters from different companies about the same debts, and the amount keeps increasing with fees etc.
So who should I put as the creditor, the original company, or the debt collectors?
Sorry if everyones a bit bankruptcy-weary!

WE don't think your stupid, we have all done the emu at some point

The easiest way to find out what you owe is to get copies of your credit files from the 3 big Credit agencies, Experian, Equifax and Call credit http://www.moneysavingexpert.com/cgi-bin/viewnews.cgi?newsid1149605730,95768,
However are you REALLY sure bankruptcy is the way forward for you? have you looked at other options? done a Statement of affairs (list everything coming in and going out)? if you have then thats great, if not you might want to post your SOA and see if there is a better option available to you.
Opps almost forgot, you need to list whoever owns the debt now as the creditor

I have printed out the letter templates to credit reference agencies from MSE and will post them off tomorrow with £2 etc. But it says in the info that they aren’t always correct.
Should I just put the amount quoted in the latest letters? Sorry if this question sounds dumb, I’ve just been trying to fill out the forms so panicking a bit.
Bankruptcy is definitely my only option, have been considering it for 3yrs. I have £45K-£55K debt and my income is only £12K net approx and I don't have any assets. Have wanted to do it for ages, but didn’t have the £500 BR fees, and none of my creditors seem keen to do it themselves, even though they kept threatening to –I guess they realised I don’t have anything to take.

If it's a debt collector, they will say if they are collecting on behalf of a company, or if they own the debt. I would just use whatever is the most recent figures they are quoting on each of the letters. Not all debts may be listed on experian etc. For example if you had an electricity debt that wouldn't be. If it goes to bankruptcy then obviously there will be delays in between the forms and the final figures, the most important thing is making sure every debt is listed, even stuff like gas or electricity if it's got to that stage.Indecision is the key to flexibility
