free wording for statutory declaration (deed poll)

please feel free to use this wording to change your name , it is accepted by all uk government organisations and only requires the signature of a solicitor or commissioner of oaths. The maximum they are allowed to charge is £5.Please remember though , that originals only are accepted so it is worth having a couple signed !
Please use exact wording , and fill in blanks as appropriate
use quality paper as you may need to send original to some organisations. send photocopies where possible and allowed
I , ( put your new name ) , of (insert your address) , formerly known as ( put your old name) and born on the (put your date of birth in word form ) , do solemnly and sincerely declare that :-
1. I absolutely and entirely renounce , relinquish and abandon the use of my former name of ( insert old name ) and assume , adopt and determine to take and use from the date hereof the new name of (insert new name ) in substitution for my former name of (insert old name )
2. I shall at all times here after , in all records , deeds , documents and other writings and in all actions and proceedings , as well as in all dealings and transactions and on all occasions whatsoever , use and subscribe my new name of ( insert new name ) in substitution for my former name of ( insert old name ) , so relinquished to the intent that I may hereafter be called , known and identified by the new name of ( insert new name ) and not by my former name of ( insert old name )

1. It is a change of name deed
2. They can charge what they like and £5.00 is usually the minimum and standard .
3. They dont have to advise you on what you have written so if it is wrong - tough.

solicitor told me max he could charge was £5 ...............EACH
so whats your point ?
p.s wording checked fully by a commissioner of oaths
so pay 40/50 quid if you want or pay £10 for two originals.
