Has anyone had Problems Upgrading with Orange & Carphone Warehouse

We have been trying to upgrade our Orange Mobile Phone contracts through Carphone Warehouse with limited success. Been in got a new phone, 4 months of huge bills later they eventually admit they made a mistake and the new tarriff hadn't been applied, Orange say they have no record of the upgrade!!! So Carphone Warehouse said upgrade again, charged us £40 to change to the new I Phone4 which seemed like a good deal, but Orange still say they haven't got an upgrade! Anyone had similar problems and how did they get them resolved?
