Free eye test and glasses with NHS exemption card?

Does anyone know where I can find out which Opticians do this?

Most do this already, high street chains such as Vison Express, Spec Savers, D&H and Boots will do NHS eye tests.

But when it comes to free lenses for the glasses, its dependant on your prescription wherever you chose to go. You need to be very badly affected by your vision, there are proper specifications but I don't know them (I just know I am 0.5 away from them and cannot get a free pair of lenses!).

Frames on the other hand, again most chain stores will have NHS frames which are free or low cost if you have an NHS exemption.

If your at all unsure, try your local Primary Care Trust or the NHS website who will be able to name specific stores in your area.
