Removing Name From Mortgage

Hi,
I have put this question to the forum once before, about 3 years ago and never really got an answer, still in the same position I'll throw it in again and hopefully someone can point me in the right direction.
My grandmother purchased a house with my uncle - her son 12 years ago and took out a joint mortgage with the Chelsea. 5 years on my uncle !!!!!!ed off and left my grandmother paying for the house herself.
In the last 7 my grandmother has had no contact with my uncle however 5 or so years back she was contacted by an NHS mental health hospital due to being his next of kin, where he was admitted and later discharged. Within a month or so she was contacted by the police where they had found him and returned him to the hospital. He again discharged himself and she has heard nothing since.
As my grandmother has been the sole payer of the mortgage she would like to remove his name from the mortgage, however the Chelsea will only allow this should my uncle agree. Of course for obvious reason she is unable to do obeyed by this.
Is there anything she can do to remove his name. I am not sure if any of the below information helps but just giving this to the forum incase it makes a difference to her case.
* My grandmother no longer works, however pensions cover living expenses.
* Remaining mortgage under £30k
I may be wrong but my understand is when the mortgage is paid off in full the deeds are handed over in both names, or am i totally wrong here?
Any lesson / Information or Guidance will be much appreciated.
