Can someone tell me why I had to pay tax out of my lump sum pension and others I know have had a tax free lump sum

Were you over the lifetime allowance?
Was it an unauthorised payment?
Was it a payment under triviality rules? (small value)
Was the whole amount taxed or just some of it?
