Bankruptcy forms

Hi guys,
I have my court appointment on Wednesday, I am filing out the forms and have a question.
I live with my boyfriend, our daughter and his dad. It is his dad's house. To help us out he let us move in rent free, we just contribute abit to bills as and when we can until we get back on our feet.
It is just me filing for bankruptcy, on the form there is a question about household income. I am pregnant and unemployed my partner is self employed but income has taken a huge hit due to recession, will I need to put in his income and provide evidence even though it is just me applying?
