what is a CSA5 FORM??????

Hi I am a PWC and I have recieved one of these forms but filling it in I got the feeling its similar to a MEF form I could be wrong, Im on Income Support and it asks all about my partner, mortgage, loans I have none of these, basically I could only fill in my name my childs name NI number and thats it, pretty pointless really, as they already have this info, can anyone shed some light please????

    Aye, but are you on CSA1 or CSA2?

    It is a change in circumstances form - it asks the same info as originally. Applies to both the PWC or te NRP. It is for CS1.
