Print the conclusion which will confirm your entitlement to 16.8 days statutory minimum paid holiday entitlement.
You could write a cover letter outlining the story so far with regard to your attempts to sort out your holiday entitlement with your employer. It would probably be a good idea to put the word Grievance in there somewhere.
Pop these into an envelope and send them to your employer by recorded delivery.
Keep copies for yourself.

You are entitled to 89.6 hours holiday a year
How many holidays do you get for bank holidays, xmas etc? How long are these days counted as at the time?
Edit based on the fact you work Mondays, Tuesdays and Thursdays only, Say your holiday year was Jan-Dec for 2010 you would have had 6 Public holiday days off at 6 hours per day so would still have 53.6 hours holiday entitlement to take.
It may also be you have had some public holidays you need to add for this.
If you have exhuasted a Holiday entitlement which seems unlikely and you had a day off then it is reasonable to expect you to work this back.
This is my best interpretation based on what you have posted.

Have you got written particulars of your employment which should, by law, include information about entitlement to holidays? (I think I know the answer to that one!)
Regarding your holiday year - without anything in writing to the contrary, it will have started on the day you started work. So for example, if you started work on 4 July 2009, your holiday year would have ended on 3 July this year and the new one started again on 4 July.0
