I contacted Best re the entry form/money off voucher issue and have had this reply:

Thank you for writing to Best. If you are emailing regarding this weeks
magazine (Issue 37) we are really sorry that the money off vouchers for the
magazine were printed on the back of the puzzle entry coupon. This is a
printing mistake on our part and we can only apologise for this. If you
have access to a photocopier, please photocopy the puzzle coupon and send
that in, or if not, please send your answers and contact information, on a
plain piece of paper to best Prize Puzzles, (37/10) The National Magazine
