When filling out bankruptcy form who do I name?

I am finally brave enough to hit this last hurdle to escape serious debt which has been hounding me for years. My main fear though is after going through the upheaval of bankruptcy companies chasing you post bankruptcy.
When fillin gout the bankruptcy form I have printed out my experian report and am filling out all the companies I owe money to. But having lost my house and moving around I have lost track of a lot of the debts which have now been passed to agencies. If I put the original creditors on my bankruptcy form does that mean in years to come the debt agencies who bought the debt can still chase me? Do I need to speak to all of my creditors to find out who the debts were passed on to?
Any help would be appreciated to stop me putting this off any longer!

When you go BR all debts will be included whether you included them on your forms or not (bar a few such as CSA and student loans). If any one tries to chase you after BR and discharge there are ways to deal with it that we can talk you through.

BR covers all unsecued debts, whethe they are mentioned on the paperwork or not. DRO are slightly different and I believe they only cover named creditors.0
