I know this is an old thread but I am a member of The Waitrose Kitchen Table. It appears they invite only a few people to take part in surveys and potentially win prizes. I have won a £150 voucher for filling in these surveys (no free samples sadly) but I was a bit worried by the long consent/release form they had me electronically sign, it talked about using photos and personal info that you *may* be asked to provide during the mini survey which asked for your details to send a prize to and of so you'd need to provide those before they agree to send the prize. I'm quite a private person and so while I don't mind my name being put down and my opinions, my photograph I do. Anyway by the end of the survey it simply said your voucher will be in the post and didn't ask me to send any photos etc. Do you think perhaps they wrote the long convoluted release form more for those who have won the top prizes like a holiday and not those who just won a voucher? I did look around for any publicity around winners of these vouchers and there were none but it's just I've never been asked to fill in a release form like this before now (granted I haven't really won much before now but still...) xx
