Credit problem

Hi,
Jeez, I have been round the houses with this one and if anyone can help, that would be great.
I recently tried to get a mortgage through Accord, but unfortunately my credit check failed.
It was because an account in my name defaulted three years ago. However, the account was linked from an address I lived at, to an address that I have never lived at. Basically, someone started the account in my name at this bogus address and maxed out the card.
So, on my Experian report, it came up as an MBNA card. But when I phoned MBNA, they said it was done through a third party. MBNA removed the linked address, but they couldn't remove the link from my name altogether becuase it was started by Clydesdale Financial Services, which is now owned by Barclays.
Unfortunately, Clydesdale Financial Services cannot find any record of the account. When I phone Call Credit and Experian, neither of them can find out any more information on this case, because the linked address has been removed from MBNA. But apparently, it is still there somewhere as Accord did a re-check on me.
So, my question is, what on earth do I do to get rid of this? Who can help me out with this as I am a little bit out of my depth.

Raise a dispute with the CRA's, MBNA or who ever registered the default will then need to get back to the CRA with proof, if not the CRA should remove the default.0
