P11D mistake?

Hi - first post so please be gentle, and thanks in advance for any help!
I left a job back in February and today received a copy of a P11D form from my ex-employer. I've never had one of these forms before (I'm certainly not important enough for a company car or an expense account!!) so I'm not entirely sure what it's for, although I understand that it's a form that records benefits in kind for tax purposes?
Looking through it, my employer has put a sum of over £400 in the section where medical benefits are recorded. However, I don't believe that this should be there. Although there was a private medical insurance scheme, I never joined it (I certainly never made any contributions and there's nothing of that kind recorded on my payslips) and obviously I never made any kind of claim. I'm a bit worried now that I'm going to end up with a tax demand and I'm not sure what I need to do about this.
I'd appreciate any kind of advice as my ex-employers weren't the nicest bunch of people and I'd like to sound like I know what I'm talking about when/if I get in touch. Is it just the case that I need to get in touch with them, point out the mistake and let them deal with it, or is it likely to get horribly complicated? Do I need to contact my local tax office? I've had a look online but most of the advice regarding these forms seems to cover filling them in, and not what to do if you receive one and it's not right.

Was the insurance scheme something where the employer automatically paid the entire fee for employees? In that case it would not have appeared as a deduction on your payslip.0 -
personally, I'd do just as you mentioned i.e. contact them to point out the error and inform HMRC. From experience, I've had a number of mistakes on my P11d over the years and sometimes it's just a typo and sometimes recording someone else's data by error.

Hi, no I don't believe it was. It was definitely an 'opt-in' thing as while I was there they changed the time at which you could join and also the price of joining went up drastically. We all got information about it and I really couldn't afford it so I never bothered joining.0
