Reclaiming Bank Charges/Obtaining Charges Info

Hello
I am about to send a letter to my bank/credit cards to request a list of the charges I have received. (http://www.moneysavingexpert.com/cgi-bin/viewnews.cgi?newsid1141050760,24632)
Now the letter I'm about to send in is the one from the consumer action group and it states I need to send in a tenner with the letter. I was just wondering do I just place a cheque in with the letter and just send to the address of my bank or do I need to be more specific and get a direct address to that 'department' of the bank?
Just a little confused, any help would be great. Thanks.

hiya, i just put mine in the envelopes and sent to registered address. made payable to name of company.

That's what I'll do then. If this is as good as people say and it works and I'm paid in full I'm undecided on whether to fork out on a BMW or a round the world trip.
