can telling your bank about a change in circumstances affect your mortgage?

im 7 years into a mortgage with Hsbc,when i took it out i was employed by a company,but for the last two years ive been working on a self employed basis but earn about £5000 p.a less than when i took out the mortgage.ive never missed a mortgage payment and have approx £60000 equity.id like to talk to the bank about possibly remortgaging to raise money for home improvments,but i am concerned that my change in circumstances might affect my mortgage can any one advise.......many thanks
