Official Receiver questionnaire

I have had a phone interview with the official receiver but would like to know what the questionaire asks as this is being sent in the post to me

Have you had your actual interview where you have discussed your SOA etc, or just the initial interview where you first make contact with the OR?:pB&SC No. 298

The only think I can think of is this, the OR is required to give or send two copies of form NTB2 to you, the purpose of which is to ensure that you are aware of a number of duties and responsibilities required. The form also points out the bankruptcy restrictions which must be observed until discharge is obtained and certain other parts of the legislation which apply to you. You must sign, date and have witnessed one copy of the form and this must be returned to the OR. And often along with this they ask for 12 months of Bank/Credit/Loan statements.

Without further details of what contact and paperwork you have had through from the OR, a few possibilities for what it might be:
- PIQB. Preliminary Information Questionnaire - used when a creditor has made you bankrupt.
- The forms/declarations found in the initial pack you are sent. Normally get them before interview, but can be after for some.
- Ethnic monitoring and/or Insolvency Service performance questionnaire found in the above ^^^^.
