A Q about defaults please

Hubby has a catalogue debt a few years ago. He was late with one payment then got a snotty letter saying youve missed the payment so we decided to pay it all off there and then job done.
Weve now discovered that theyve given him a default in 2004.
Hes never had a letter saying this. Has never signed anything relating to this and we only found out by getting his credit file.
Weve claimed back these admin charges in full by the compnay who send out the £180 charges in gift vouchers. But where do we stand with this default. Can we get it removed. I know I shall have to write to them but what do I say epseically now as theyve paid up in full for the admin charges
