Self-referral for physiotherapy?

Just been looking on the NHS website and it seems to think that I can do a self-referral for physiotherapy on the NHS.

I've previously had some work done on the NHS but it's deteriorated (back/shoulder/neck) to where I am now in a lot of pain.

Has anyone done it or has any idea how it works? I will ring GP practise tomorrow but mine are not the most helpful of practises. Thanks all

    The way ours works is that you turn up to an open session, wait around until you can be seen, then have a brief assessment. If they think they can help they book you in. I was just given an exercise sheet and told to phone if that didn't help enough (it did).
