Just worked OK for me but thought the form a bit odd. WARNING - This offer looks very much like a scam.
The offer site (agrofarming.com) is just a copy of the site at agrofarm.org, with just a small change to the logo text and the addition of the free pen/notebook offer. The Whois info shows the two sites have very different registrations, so I think anyone who's applied for this should expect a mountain of SPAM.
