I am filling in the form on moneyclaim and am stuck on the 1st page!!! I am claiming against capital one and just wondering what i put in the particulars of claim box and if i put yes or not to next questions of human rights and reserve the right to claim interest!

No to human rights, yes to reserve the right to claim interest (you must state as stipulated under s.69 of the County Courts Act 1984 I believe), in the particulars of claim box but that you're claiming punitive charges back as they aren't liquidated damages.
No offence, well yes actually slightly offensive, but it sound like you haven't bothered to read the FAQ on the CAG website - https://www.consumeractiongroup.co.uk
You do realise this has the potential to go to court, so you really should read about the legal process you're about to undertake before being so gung-ho and filling in legal forms. /rant
