Passed my test but can't find counterpart.

Just to clarify, can I send away only my practical certificate and provisional photo ID, or do I need to fill in a form given that I can't find my counterpart?

The plastic part is not valid without the paper part, the two should always be carried together, that's why they give you a little plastic holder to keep them in. Since you are getting a totally new license, I'm sure it wouldn't be a problem just to send the plastic part and tell them you lost the paper part.0

NEVER carry your paper part on your, it is so easy to loose and you don't need it unless you are in trouble with the police etc, at which point you have like a week to two to produce it.
its just common sense that if its important and easy to loose or damage, keep it at home.
i think you may have to fill in a form, but as people have already said, your old counterpart will change when you get a new one anyway, so you might be ok.0
