Mortgage nightmare

Any advice?
My sister and her husband are paying a very high mortgage which due to serious illness and therfor drop in income they are now sruggling to find. They will have to pay a ridiculous (!! ) amount of money if they want to reduce the amount of money they pay monthly. Is there any way this can be avoided?
Any ideas welcome!

You haven't given much in the way of details there...

How high is the mortgage? How much do your sister and BiL earn? Is the reduction in earnings likely to be temporary or permanent? What is the "ridiculous" amount of money for - switching lender, switching product...?
