NTB2 form

sorry, probably a dumb questions. got my NTB2 form off my OR and i have to sign and date it. where its says 'witnessed by' and 'address', does the witness print and sign their name and their address? or just their signature and my address?

    The witnessess address, so it could be verified if it was ever called into question

    Does everyone have to fill this form in?I am not telling any family etc as I want as few people as possible to know about it so would be stuck for a witness.

    Same prob, I'm awaiting this form and haven't told anybody about BR and don't want anyone to know.
    Wish there was a 'witnesses-r-us' you could use lol.

    Everybody has to fill this in, but the witness can be anyone - if you are close to your OR's office, then the examiner can witness the form for you.
