Couple of questions for completing my BR Petition

I'm aiming to present my BR Petition next thursday (April 1st of all days). I'm busy completing it now, but I need a couple of questions answered regarding the creditors section of the form.

1. Do I state the original creditor eg Lloyds or do I state the current creditor who has taken over from Lloyds which in this case is SCM Solicitors?

2. I am self-employed and have missed the January deadline for paying my income tax, should I add this debt to my petition?

    Quote lloyds and original account number and put note on with who debt has been passed to, use seperate paper if necessary and attach any correspondence for the OR.

    Yes put Inland Revenue debt on petition.

    If it is estimated any loss will go into your BR and so will any refund.
