Mortgage problems with the halifax

CAN ANYONE HELP ME???? HALIFAX PROBLEMS!!
I Instructed a Solicitor to remove my Mothers name from our mortgage ( she was guarantor for me & my husband) after she passed away. this was done using an AP1 form and the copies sent to the Halifax ( our mortgage lenders). I now want to release some equity, and the Halifax have been dragging their heels since Jan 2010 as they say they never received a copy of the application, therefore, there has been an illegal transfer of property!!! I have spoken to them non stop, as well as my conveyancer and am getting nowhere fast. The bank are now saying my husband has never been on the mortgage!!!!! ( although they have sold him TMPP insurance!!!) Is this even legal?
Also, in August 2008, at a mortgage review, I was asked if I wanted to combine my insurances to one policy ( for myself only) to which I agreed, and the halifax didn't cancel my old policy, ( so I paid twice for the same cover until Jan 2010). I am having to chase my refund on this still.....have I been missold insurance?
Any advice would be welcome as I'm at my wits end with the stalemate situation.
