i'm doing my form and i'm a little confused 
do i list my bank accounts (using overdraft on both) as unsecured creditors in section 4, or just include them in the banks/creditcard section?
also, what address do i use for my bank accounts, my branch or the customer service address??

Bank accounts go in both sections. As to address, whichever you can get your hands on; it doesn't matter as any correspondence will be passed to the relevant department anyway. Just ensure that your account numbers are correct.0

Another question. on the online form it mentions under step 13, supplementry information, 'Previous Residential Addresses' it asks to provide details of any former addresses you have lived at, where debts or liabilities may still be unpaid or unsatisfied relating to your petition for bankruptcy. I moved a few months back, but all my creditors know my new address so do i need to put my previous address down?0 -
Yes. It asks for your previous address, so supply it.
It's really so that the OR can cross reference everything.0
