End of fixed term but on debt management plan

My mortgage is with The Chelsea and has 2 years to go until our fixed rate ends. Since tsaking it out though we have got ourselves in a right mess and are now on a DMP with the CCCS so our credit rating is shot to pieces. We also have a first plus loan secured on the house and one of our creditors got a charging order for an unsecured loan.
I was just wondering what options the chelsea are likely to present us with at the end of our term?
