JT, you're not going to get anywhere with posts like this.

If you are serious about needing help with the questionnaire, your first port of call should be to contact one of the site administrators or go through the "contact us" link to ask if they can check the website link you are posting and to assist with your request for board members to complete it.

Once board members are sure this is not a spam or virus site, you may get more feedback - without this though, your posts will probably be ignored and considered spam.
