    Which i'm sure it didn't in the original OP?

    I don't remember seeing that either.

    Just tried again and all I get now is

    The form webpage is back up again OK but they have certainly changed the wording to say catering establishments only since the freebie was first discovered.

    I think it's a game of hide & seek, and they keep hiding the webpage from us. Must be frustrating for them when an MSEer finds it again!
