Yes you also access that form by clicking on a 'Health Care Professional' link on the Vaseline homepage.

Thanks toni_ I work in a hospital, so put my work address + email.

I clicked on the News page out of interest and their last News Update was for Spring 08 - not much has happened in the last two years then at Vaseline!
