From personal experience I would say let them know now - I had been in a serious car accident 3 years ago (and was off a total of almost 9 weeks over several occasions in a year relating to the back injury sustained) but when I applied for a job with a university I didn't state total days off. I was offered the post but when they checked with my then employers (who made me redundant only weeks later!) they withdrew job offer and said I could not reapply for 3 years (only another 18 months to go ho hum). So now I am completely honest but have only had 3 interviews in last 12 months! You just can't win!

I would say the worst case scenario is when your current employer finds out not only that you want to leave, but that they are stuck with you because you lied on the application.
I will make 100% sure I count every single day/hour/minute of sickness from now on.

As I said, to add insult to injury (so to speak) I was made redundant at almost the same time as the job offer withdrawal so have been without a job for 18 months now with little hope of getting another one because of sickness levels (I dread seeing that question on application forms now - had problems even registering with employment agencies yet I "temped" for almost 15 years prior to this employer with no problems and excellent references)
