Outstanding Services Balance - dealing with Fredrickson Int Ltd

Hi everyone
I recently left Tiscali (now TalkTalk) and had an outstanding balance which the main office passed to Fredrickson International Limited. The outstanding balance is £235 which unfortunately I cannot pay in one go.
Fredrickson have contacted me a few times, each time ignoring my return letter but now they have forwarded me a form requesting certain details, including my employer details and bank account information.
I am not comfortable sharing this information with them, do I HAVE to give them this information? They tell me this form must be completed fully before they will consider their agreement with me.
Any help is always greatly appreciated.

They've given me so many days to complete this form. After that they'll continue their previous hounding with phone calls and last notice letters.
Any ideas greatly appreciated whether I should complete the above form? Or shall I send them another expenditure form, without personal details, in hopes they comply0 -
You dont have to give them either of the things they have requested , the cheeky sods....there are templates etc for this sort of thing on here but someone far better at it than me will hopefully come on the thread soon mate0

I have something more appropriate for them, and no I don't mean a middle finger, lol. I'll post that.
