Needs some help on OSP & holidays - work for High ST Bank

I 'm hoping for some help and advice.
For a little big of background - I work for a High St Bank and have worked for them for 4 years now. I moved nearly 2 years ago to a new area and team - who are abs horrible and very unhelpful.
I had a bad break last year and have been off work since 3rd August.
Work have been paying me Occ Sick Pay (ie full wage). People have different contracts at work and I am on a flexible contract which means I can change my hours, compress them work flexi etc (which they do not like as the others I work with are on non-flexible contracts).
Anyone any idea when this OSP would run out? I believe work are meant to write to me to let me know it is running/run out. The reason I ask is that I have just been signed off for another 4 weeks and am worried I wont be bringing a full wage in (I am the main earner). We do get a little working tax credit and child tax credits.
I cant find my contract and have asked for contact points at work but not had any replies as yet. I thought I would only get 16 weeks - but at the moment it has been 26 weeks. I also thought I read somewhere - that OSP can be for 28 weeks.???
Also - as I had my accident when I did - my main holidays were booked for Aug, Oct & Dec. They would total approx 12-15 days (cant remember the exact days)
I have been told by work that I have lost them!! I cannot get paid them (as its at the managers discretion) and cant carry forward.
However - no matter when I go back this year, I will get a full years worth of holiday??!?!?!
Is this right?? I know the bank I work for tends to "make its own rules up" and they go on budgets - so I am disappointed to have lost these holidays (ie the manager made a decision not to give them to me)!!

I'd imagine it will be 26 weeks full pay and 26 weeks half pay absolute maximum. Some employers will have this on a sliding scale dependent on service
Tbh all you can do is ask HR, there is no point in ignoring it as if you are overpaid they will ask for it back.
As for holidays. It often depends on policies in place. I am surprised they are saying you are losing them but you probably should have contacted them before they expired to sort it out.0 -

I contacted them at the beginning of December when I knew I was signed off for at least another 6 weeks. Thats when they told me that I had lost them!!0
