Help with application form - not sure what to put!

Ive got an application form for a job that sounds perfect for me, but ive never seen a form like it most of the jobs ive had you just went for a interview, looked at cv and got the job on the spot.
Here are the sections that Im stuck on:
knowledge
skills
attitude
additional notes
theres big boxes to write your answers in and im a bit worried that ill keep repeating myself or end up just re writing my cv on it.
PLEASE HELP!

there's normally a job pack, and within it there should be a job description and role specification which should give clues as to what they're looking for.0

Skills is easy - put down things like your computer skills, typing speed, ability to work under pressure, that sort of thing.
Knowledge - do you have knowledge (but perhaps not enough knowledge to make it a skill) of databases, word processing, how to work a wood turning machine, etc.
Apologies, you haven't said what type of job you are applying for.
Attuitude - are you the type of person that works to ensure that the job is done - are you flexible in working hours ( i.e. kind of want to stay until you are sure that a client is happy (e.g. a call centre), do you have a can-do attitude, willing to go the extra mile to ensure that a client/customer is happy.
Give examples - I work in a shop part time and tonight a guy came in - wanted lentils - we had run out in our shop. Rang the two other shops, one had the lentils, one didn't. Arranged to get some lentils put aside for the guy so he could go and collect them in case there was a rush in lentils!!!
Additional notes - difficult. Worth repeating perhaps a particular skill that you have - shorthand, operating a machine, writing, that sort of thing.
Are you asked to also send in your CV? If you are, then it is kind of hard to not repeat yourself. If not, then take out the succint parts of your CV, the parts that you are confident of and that you have particular skill in, and put them on the form.
