If there is a person named in the job adver adress it to them. Only use Dear sir/madam or to whom it may concern only if there is no name on the advert and after doing so basic research (phone the company, website etc) you have not been able to get a name to address it to.

If the adverd reads like this "apply to Joe Bloggs in writing to Company Address" don't assume they are Mr Bloggs, always phone and ask who it needs to be addressed to and their title (mr, miss, mrs etc). If they won't tell you address it to Joe Bloggs on the address on both letter and envelope.
I used to work in HR and it always drove us mad when people didn't take 5 minutes just to find something as basic as that.

If there's a name specified, always address it to them. It shows that you've paid attention and you're not just using a generic term such as Dear Sirs etc.

For example if the name says Mrs Sally Smith, address the letter to Mrs Smith. If it says Mary Jones, address the letter to Ms Jones... etc... apply the relevant term.
