Medical Questionnaire

Hi All
Bit of a strange one not sure if this thread is the right place for it (if wrong place can someone please direct me to a more appropriate thread please
).
I need a medical questionnaire drafted for my son's biological father to complete. We have had discussions through a solicitor and he has agreed to complete it but I now dont know where to access one from. A friend has shared a insurance company one withe me which I can adapt but I wondered if anyone had any better ideas.
My son is 4 and is under the consultant for bowel issues and diary intolernace his biological father has IBS and so I wanted details of his diagnosis to assist my son's treatment etc. Also his father and half sister both wear glasses and I would like details of this too. He has agreed to answer but I just want to now obtain the information in the best way so as not to have to go back to him again and have to ask for more.
