I could not remember the dates so what I did was put down the dates I actually took them out. For CC I put the date down when I got them, and for O/D`s I put down the dates I opened the bank account. I got all these dates from my credit reports, otherwise I would have been totally lost. If you don`t have these to hand just guess as best you can, and explain this to the OR.

Put a best guess estimate as to when you took the loans, CC's and OD on. Then make a note that that is what you have done. The OR will find the dates he needs, it would just make his life easier if you did know the dates, but you won't be the first and you won't be the last so don't panic to much.

  • You can get a copy of your credit report from Experian by sending them a £2 postal order and it will list all your debts, when they were first incurred, and how much is still owing, as well as addresses for all your creditors, very handy for filling out the forms.

You can download a copy of the form from here:

You don't need to be that accurate. I forgot to put the dates on mine and the clerk at the court said to just put anything down as she could not process the forms without that information so I just guessed. It didn't make the slightest bit of difference.
