affidavit template uk (urgently needed)

Hello,

firstly i am 100% sorry if this is the wrong section. i could not find where else to put it.

I made the silly misstake of pawning a bracelette to cash converters and lost the receipt.

If i go into a solicitors they want £100 to do this.. i spoke to CAB and they gave me the number of one that if i download a template they will sign in.

but i dont really understand what it is.

i need to add the information to it as follows

transaction number

date of transaction
money owed
money giving to me for bracelette

and item

please help. this has got to be in by 5. im such a fool
