Resignation letter

Im trying to a resignation letter, ive done this bit so far, is this ok, i only did a letter a few months ago and i cant even remember what i typed for that one

Please accept this letter as notice of my resignation from XXXXXXXXXX.
I am giving you 1 weeks notice so my last day here would be 21/06.
I would be grateful if you could forward my P45 to my home address.
I wish the company success in the future and hope you would honour my request.

    Something like that... personally I would have worded it slightly different
    Please accept this letter as formal notice of my resignation. I am aware that I am required to give a X week notice period and therefore my last working day will be Y.

    I have enjoyed my time working with you and the company and wish you all the best for the future

    You could add a bit in about prefering a shorter notice period if it is agreeable to them if you do want shorter notice.

    Wow! - there really is a site for everything isn't there?!

    All I need now is the "the treasure is buried here..." site.
    The atmosphere is currently filled with hypocrisy so thick that it could be sliced, wrapped, and sold in supermarkets for a decent price and labeled, 'Wholegrain Left-Wing, Middle-Class, Politically-Correct Organic Hypocrisy'.

    regarding the resignation letter i want to add something like "any outstanding monies payed into my account", how would i do that

    None of the templates mention holiday! I would add the sentence 'I would be grateful to receive your calculations of any holiday pay that I am due to the calculated leaving date'. Only reason for not including this sentence is if you think you have 'overspent' on yr holiday, in which case you don't want to remind them :p

    What outstanding monies are you likely to have, to be paid into your account? You could say something like 'You will also be aware that you owe me £xxx.xx for expenses/overtime/whatever, and I would be grateful if this money could be paid, together with my final salary, direct into my account.'
    Ex board guide. Signature now changed (if you know, you know).
