Well I don't know about templates but when I have closed down accounts I have just written a letter along the lines of:
Dear Sirs
Re: Account Number xxxxxxxxxxxx
Please arrange for my account number, as mentioned above, to be closed with immediate effect.
My balance is zero and, as far as I am aware, I have no outstanding charges or fees. If this is not the case then please contact me as soon as possible with details.
I enclose my destroyed card/cheques <this is optional>
or
I will be destroying my card/cheques today.
Please reply to my instructions to inform me of the account closure.
Thank you.
Yours faithfully
Ms N.O. MoreSpend (I made that bit up)
I hope that helps!Bank Balance: In the black for the moment.

Having just cleared my first CC account, i was thinking along the same lines as you. I really want to write and close the account, so that I am not tempted and so that I can have the satisfaction of saying 'I don't need you anymore thankyou'. I am going to use the letter that some kind poster posted above, cut the card in half and send it to them.

Be fully prepared for questions such as "Do you mind if I ask why you want to close / terminate the account", but don't think they're doing it to try and guilt trip you or catch you out - coming from a customer service background it's often geared from the "every complaint is a suggestion to improve our service" believe it or not (and I work for ntl, so feel free to through sharp objects at me...)
I've answered such questions with "I just don't need the service anymore", and that's normally enough (although sometimes a magical 'offer' will appear)
I once got a letter from Capital One saying "We're sorry you no longer want to be with us but thank you for the time you spent with us", which was kind of nice, I suppose?

Why don't you just phone them up?
Letters have a habit of disappearing!
Whenever I do it I never get hassled or anything. They ask why you are closing and all you say is I'm trying to get my finances in order and improve my credit rating. Sometimes they will even offer you a decent balance transfer deal to keep it open.
They then just tell you how it's your responsibility to cancel any regular payments you may have setup on it and to cut it up.
Make sure you ask them to send you a letter to confirm it's closed though.

Hi posting.
I don't think you have to send them the card. I have 'shredded' mine and put them in the bin before (go through the chip bit for sure!) just tell them you are destoying it.
I have also sent shredded 'bits' back to them as well.
It doesn't really matter as long as they have been told you want to close the account.
I agree with SS. You can ring them up - but I would follow it up with a letter and I would want one back in return.
